Health Services

All students attending St. Michael the Archangel School must have received immunizations as mandated by the Commonwealth of Pennsylvania. Because regulations have changed over the past few years, read the following information to determine what the immunization requirements are for your child. Your child must have the updated required medically appropriate immunizations or a plan to complete those immunizations signed by the physician. If your child does not have proper documentation or a signed plan they are at risk of exclusion from school. If your child has already received the immunizations please provide documentation to the office.

If your child does not have the immunizations you need to have your primary care provider fill out and sign the Immunization Card/Medical Plan with the dates your child will be receiving the missing required immunizations. The proof of missing immunizations or the immunization card/medical plan must be submitted within the first 5 days of school.

Please keep in mind that if your child is missing any immunizations, they must receive the first dose before the first day of school or they will not be allowed to attend. At that time they will be considered a “provisional” student until all immunizations are completed.

The Commonwealth of Pennsylvania has minimum immunization requirements for all students schoolvacrequire 

ALL STUDENTS ARE REQUIRED TO HAVE:

  • 4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)
  • 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**
  • 2 doses of measles, mumps, rubella***
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or evidence of immunity.

* Usually given as DTP or DTap or if medically advisable DT or Td

** A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose

*** Usually given as MMR

FOR ATTENDANCE IN 7TH GRADE:

  • 1 dose of tetanus, diphtheria, accelular pertussis (Tdap)
  • 1 dose of meningococcal conjugate vaccine (MCV)

Heath Screenings

Students will undergo health screenings throughout the school year including:

Exam Grade
Dental Kindergarten or First, Third, and Seventh*
Hearing Kindergarten, First, Second, Third, Seventh
Height and Weight Kindergarten through Eighth
Vision Kindergarten through Eighth**
Physical Exams Kindergarten or First, Sixth
Scoliosis Sixth and Seventh

**First graders receive color, depth perception, and extreme far-sightedness screenings

Any medication that needs to be given during the school day must be given to the main office for safe-keeping. A medication in school form must also be completed by the doctor in order for the district nurse to administer the medicine. Students are not allowed to bring any type of over-the-counter medication to school without written permission from a doctor’s office or parent.
